Terms, Conditions & Cancellation Policy
1. Appointment Scheduling: The best way to reschedule or cancel is via the website. Do not use social media to reschedule, instead use the website and if necessary text us at 510.224.3247. In the unlikely event the website is not working, then call the office. We require the you give us at least 12-hour notice so that we have the opportunity to offer your appointment to another patient via the website.

2. Cancellation and No Shows: A “No-Show” is someone who misses an appointment without notice. No-Shows inconvenience patients that are in need of our services. A failure to cancel a scheduled appointment without 12-hour notice will be recorded in the patients file and the full price of the visit will be charged. All fees are paid with the card on file. Multiple No-Shows may result in suspension of care.
Understand that cancellation & no show fees have nothing to do with co-pay or deductible and in fact cannot be billed to an insurance company.
Memberships and packages may be refunded with the following notes. Before its expiration, refunds will be prorated at the highest fee, known as the 'pay as you go' fee. Refunds are not given if the package or membership has expired.
3. Insurance: We do not work directly with insurance. Patients are responsible for any co-payments, deductibles, or non-covered services. You may request a superbill to send to your insurance company.

4. Payment: Payment is due at the time of booking with credit, debit or HSA cards. No cash is accepted.

5. Confidentiality: Your privacy is important to us. All patient information and records are kept confidential in accordance with applicable laws and regulations. See open adjusting for incidental disclosure.

6. Treatment Plans: Dr. Debra Foxfern DC will create a personalized treatment plan tailored to your specific needs and goals. It is essential to follow the recommended treatment schedule for optimal results.

7. Open Adjusting: Adjusting sessions have two or more tables per room. It involves several patients being seen in the same room at the same time with the Doctor rotating between clients. Patients are within sight of one another and some ongoing routine details of care are discussed within earshot of other practice members.
This is NOT the environment used for taking patient histories, performing examinations or presenting reports of findings. These procedures are completed in a private, confidential setting. This is not the case for booths, fairs or festival consults.
We are requesting this authorization with respect to what is known as an “incidental disclosure” of health information. It is our view that the kinds of matters related in an open adjusting environment are incidental matters.

8. Emergency Situations: In case of a medical emergency, please call 911 or visit the nearest emergency room.

9. Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of the state of California.

10. Changes to Terms: Dr. Debra Foxfern DC reserves the right to modify these terms and conditions at any time. Changes will be effective immediately upon posting.
